For Basketball and volleyball (birthdates 2002 through 1994) – please email mike@olscyo.com  to inquire about openings.   

For all other sports, please print out a registration form, complete, sign and mail or drop off at parish office with a check or money order (NO CASH) made payable to OLSCYO. Mailing address for parish office is 258-15 80^th Avenue, Floral Park, NY 11004.  Parish office is located on 259^th Street side of the school building.

You may also stop by the parish office during office hours to register

Every family pays the family fee at the top of the form.  After family fee is paid, it covers all children in that household.  Individual sport fees for each child must be calculated in the payment as well as uniform fees if new to CYO this year.

REMINDER:  One can participate in our basketball and volleyball programs though last year of high school.
